Resumo Introdução: A violência contra idosos é um fenômeno crescente, ocasionando prejuízos à saúde, com diferentes desfechos e consequências às vítimas. A chance de idosas sofrerem-na no âmbito familiar supera a dos homens, sendo o gênero um fator de risco considerável. Objetivo: Analisar a compreensão da violência contra pessoas idosas segundo mulheres gerontes. Metodologia: Pesquisa descritiva com abordagem qualitativa desenvolvida com 22 idosas de uma comunidade no estado da Paraíba, Brasil, escolhidas por conveniência. Utilizou-se para coleta de dados entrevistas semiestruturadas, processadas pelo software Iramuteq, com posterior Análise de Conteúdo. Resultados: Foram evidenciadas cinco classes: ciclo de violência; rede de apoio ao idoso vítima de violência; Vivência de situações violentas; violência financeira; e simbologia da violência na sociedade, as quais denotam compreensão da violência envolvendo os diferentes tipos. Apoiam-se nos fatores da vivência familiar, cultura e outros, consubstanciando o profissional de saúde como fundamental para o desfecho. O gênero influenciou no que concerne ao olhar lançado sobre a violência física e psicológica, bem como na relevância dada às equipes de saúde para identificação de ocorrências e prevenção de possíveis danos. Conclusão: Os diversos tipos de violência contra a pessoa idosa foram reconhecidos, incluindo fatores individuais, comunitários e sociais no ciclo violento. Além disso, associaram o envelhecimento a maior suscetibilidade para sofrer violência, independente da tipologia. Destaca-se a potencialidade do serviço de saúde na assistência à pessoa idosa vítima de violência, elucidando casos e atuando precocemente para interrupção dos ciclos perpetrados, exigindo a necessidade constante de atualização profissional para lidar com situações detectadas.


Resumen Introducción: La violencia contra las personas adultas mayores es un fenómeno creciente, que causa daños a la salud, con diferentes desenlaces y consecuencias para las víctimas. La posibilidad de que las mujeres adultas mayores la sufran en el ámbito familiar supera la de los hombres, siendo el género un factor de riesgo considerable. Objetivo: Analizar la comprensión de la violencia contra las personas mayores según las mujeres adultas mayores. Metodología: Investigación descriptiva con enfoque cualitativo desarrollada con 22 mujeres adultas mayores de una comunidad en el estado de Paraíba, Brasil, elegidas por conveniencia. Para la recolección de datos, se utilizaron entrevistas semiestructuradas, procesadas por el software Iramuteq, con posterior análisis de contenido. Resultados: Se evidenciaron cinco tipos de violencia: ciclo de la violencia, red de apoyo población adulta mayor víctima de violencia, experimentar situaciones violentas, violencia financiera y simbología de la violencia en la sociedad, que denotan la comprensión de la violencia de diferentes tipos. Estas ideas están respaldadas en los factores de la experiencia familiar, la cultura y otros, donde la persona profesional de la salud se identifica como fundamental para el cuidado y apoyo. El género influyó en la mirada lanzada sobre la violencia física y psicológica, así como en la relevancia dada a los equipos de salud para la identificación de sucesos y la prevención de posibles daños. Conclusión: Se han reconocido los diversos tipos de violencia contra las personas mayores, incluidos los factores individuales, comunitarios y sociales en el ciclo de violencia. Además, asociaron el envejecimiento con una mayor susceptibilidad a sufrir violencia, independientemente de la tipología. Destaca la potencialidad del servicio de salud en la asistencia a la persona mayor víctima de violencia, mediante la identificación de casos y la actuación temprana para la interrupción de los ciclos perpetrados. De manera que, se evidencia la necesidad constante de actualización profesional para hacer frente a situaciones detectadas.


Abstract Introduction: Violence against the elderly is a growing phenomenon, causing damage to health, with different outcomes and consequences to the victims. The possibility of elderly women suffering it in the family context surpasses that of men, with gender being a considerable risk factor. Objective: To analyze the understanding of violence against the elderly according to elderly women. Method: Descriptive research with a qualitative approach developed with 22 elderly women from a community in the state of Paraíba, Brazil, chosen for convenience. The data collection was based on semi-structured interviews, processed by the Iramuteq software, with subsequent Content Analysis. Results: Five classes of violence against the elderly were evidenced: cycle of violence; support network for the elderly victims of violence; experience of violent situations; financial violence; and symbolism of violence in society, which denote an understanding of violence involving the different types. They are based on the factors of family experience, culture, and others, placing the health professional as a fundamental element for care and support. Gender influenced the perspective on physical and psychological violence, as well as the relevance given to health teams for the identification of occurrences and the prevention of possible damage. Conclusion: The various types of violence against the elderly have been recognized, including individual, community, and social factors in the violent cycle. In addition, they associated aging with greater susceptibility to suffering violence, regardless of the typology. It highlights the potential of the health service in assisting the elderly victim of violence, elucidating cases, and acting early to interrupt the cycles perpetrated, requiring the constant need for professional updating to deal with detected situations.

OBJECTIVES: The prevalence of street drug abuse is difficult to establish in women because of stigma associated with the practice. The main objective of this study was to analyze possible differences between men and women in a sample of patients attended for emergencies due to acute poisonings. The secondary aim was to identify variables associated with severe poisonings defined arbitrarily as requiring intensive care for more than 12 hours after hospital admission. MATERIAL AND METHODS: Retrospective study in 2 hospital EDs. We included patients over the age of 18 years attended for street drug poisonings between July 1, 2020, and July 31, 2023. Epidemiologic, clinical, and laboratory variables were analyzed. RESULTS: A total of 1032 patients were studied; 191 (18.5%) were women. The mean (SD) age was 35 years, and the prevalences of acute mental illness and HIV infection were high at 32.2% and 35.7%, respectively. Drug use was recreational in most cases (90.9%). Cocaine, alcohol, and methamphetamines were the substances most often used. Multivariate analysis showed that the factors associated with the seriousness of poisoning were age, with an odds ratio (OR) of 1.03 (95% CI, 1.01-1.05; P = .003); HIV (OR, 2.10; 95% CI, 1.29-3.41; P = .003); use of benzodiazepines (OR, 3.48; 95% CI, 2.14-5.66; P .0001); and suicidal ideations (OR, 1.82; 95% CI, 1.25-3.79; P = .004). CONCLUSION: Differences in poisoning characteristics in women were found, probably related to the younger ages of men in the sample and their higher frequency of alcohol consumption. Public health policies and campaigns to prevent drug abuse should take gender differences into consideration in order to adapt messages to the target populations.

El objetivo principal fue analizar las posibles diferencias de las intoxicaciones agudas (IA) según el sexo en una muestra de pacientes atendidos en dos servicios de urgencias hospitalarios (SUH). El objetivo secundario fue identificar las variables asociadas a las intoxicaciones graves, definidas de forma arbitraria como las que requerían una atención intensiva médica de más de 12 horas y posterior ingreso hospitalario. METODO: Estudio retrospectivo en dos SUH que incluyeron pacientes mayores de 18 años atendidos por IA por drogas de abuso, en el periodo comprendido entre el 1 de julio 2020 y el 31 de julio 2023. Se recogieron variables epidemiológicas, clínicas y de laboratorio. RESULTADOS: Se incluyeron 1.032 pacientes, un 18,5% (191) mujeres. La edad media fue de 35 (DE 10) años, con elevada prevalencia de enfermedad mental aguda (32,2%) e infección por VIH (35,7%). El principal motivo de consumo fue lúdico (90,9%). Las principales drogas de abuso fueron cocaína, alcohol y metanfetaminas. El análisis multivariado mostró que únicamente la edad (OR: 1,03, IC 95: 1,01-1,05, p = 0,003), el VIH (OR: 2,10, IC 95: 1,29-3,41, p = 0,003), el consumo de benzodiacepinas (OR: 3,48, IC 95%: 2,14-5,66, p 0,0001), y la ideación autolítica (OR: 1,82, IC 95%: 1,25-3,79, p = 0,004), se asociaron a gravedad de la intoxicación. CONCLUSIONES: Las IA por drogas de abuso en mujeres presentan algunas diferencias en relación a las de los hombres, ya que son más jóvenes y asocian consumo de alcohol con mayor frecuencia. Las campañas de prevención y políticas sanitarias sobre el uso de sustancias deberían tener en cuenta las diferencias en el consumo según el sexo para adaptarlas a la población a las que vayan dirigidas.

Background: Higher alcohol use and military sexual assault (MSA) are associated with increased risk of death by suicide. Risk for death by suicide is rapidly increasing among females, who report higher rates of MSA, yet actual death by suicide and alcohol use are higher among males. It is not well understood whether higher alcohol use confers greater suicide risk in male or female service members and veterans who have experienced MSA.Objective: To determine whether the association between alcohol misuse and suicide risk was moderated by biological sex in a sample of male and female service members (N = 400, 50% female) who reported MSA.Method: Participants completed surveys of alcohol use and suicide risk as well as a demographic inventory. Linear regression with an interaction term was used to determine if suicide risk differed by sex and alcohol use severity after accounting for discharge status, sexual orientation, and age.Results: Average scores on the suicide risk measure were consistent with an inpatient psychiatric sample and scores on the AUDIT-C were indicative of a probable positive screen for alcohol misuse. Suicide risk was most pronounced among males who reported higher levels of hazardous alcohol use. A sensitivity analysis examining suicide risk by sex and screening results for alcohol misuse (positive/negative) showed that men with a probable positive screen had higher suicide risk.Discussion: The current study provides novel findings on suicide risk among survivors of military sexual violence by including both male and female survivors. Interventions to decrease suicide risk following MSA may consider alcohol reduction strategies, and optimizing these interventions in males. Engaging military culture at both the US Departments of Defense and Veterans Affairs to encourage more healthy alcohol consumption may mitigate this public health concern. Future research may consider how country of origin relates to these associations.

Background: Individuals suffering from PTSD following childhood abuse represent a large subgroup of patients attending mental health services. The aim of phase-based treatment is to tailor treatment to the specific needs to childhood abuse survivors with PTSD with a Skills Training in Affective and Interpersonal Regulation (STAIR) phase, in which emotion dysregulation and interpersonal problems are targeted, and a trauma-focused phase.Objective: The purpose of this study was to compare STAIR + Eye Movement Desensitization and Reprocessing (EMDR) vs. STAIR + Narrative Therapy (NT) as treatments for PTSD following childhood-onset trauma in a routine clinical setting.Method: Sixty-eight adults were randomly assigned to STAIR/EMDR (8 STAIR-sessions followed by 12 EMDR-sessions) or STAIR/NT (8 STAIR-sessions followed by 12 NT-sessions). Assessments took place at pre-treatment, after each treatment phase and at 3 and 12 months post-intervention follow-up. Primary outcomes were interviewer-rated and self-reported symptom levels of PTSD. Secondary outcomes included symptom levels of depression and disturbances in emotion regulation and interpersonal skills.Results: Multilevel analyses in the intent-to-treat sample indicated that patients in both treatments improved substantially on PTSD symptom severity (CAPS: d = 0.81 to 1.29; PDS: d = 1.68 to 2.15), as well as on symptom levels of depression, anxiety, emotion regulation, dissociation and interpersonal skills. Effects increased or were maintained until 12-month follow-up. At mid-treatment, after STAIR, patients in both treatments improved moderately on PTSD symptom severity (PDS: d = 1.68 to 2.15), as well as on symptom levels of depression (BDI: d = .32 to .31). Symptoms of anxiety, emotion dysregulation, interpersonal problems and dissociation were not decreased after STAIR. There were no significant differences between the two conditions on any outcome.Conclusion: PTSD in adult survivors of childhood interpersonal trauma can effectively be treated by phase-based interventions using either EMDR or NT in the trauma-processing phase.Trial registration: ClinicalTrials.gov identifier: NCT01443182..


The study directly compares Skills Training in Affective and Interpersonal Regulation (STAIR) followed by either EMDR or Narrative Therapy in the trauma-processing phase in routine clinical setting.The brief phase-based treatment was found to be effective in reducing both symptoms of PTSD as well as emotion regulation and interpersonal problems in survivors of childhood abuse.Posttraumatic Stress Disorder in adult survivors of childhood interpersonal trauma can effectively be treated by phase-based interventions using either EMDR or Narrative Therapy in the trauma-processing phase.

En prisión existen programas específicos de tratamiento para condenados por agresión sexual a menores; sin embargo, quienes además tienen pedofilia, requieren una atención específica. El objetivo del presente estudio es doble: primero, ofrecer una propuesta de intervención específica para pedofilia en el entorno penitenciario basada en la evidencia científica disponible y complementaria al Programa de Control de la Agresión Sexual y después, justificar dicha propuesta a través del caso de un interno con pedofilia. En concreto, proponemos una intervención individual y centrada en personas con pedofilia; trabajando la baja autoestima, evaluando la ideación suicida y reestructurando las distorsiones cognitivas con menores, facilitando la creación de relaciones personales funcionales con adultos, evaluando la polivictimización pasada y su posible influencia sobre la conducta sexual posterior, proporcionar a los internos psicoeducación sobre pedofilia; atender de forma específica las fantasías sexuales con menores, evitando las conductas disexuales y la eliminación del consumo de material abusivo.(AU)


There are specific treatment programs in prisons for those convicted of sexually assaulting minors; however, those who also have pedophilia require specific attention. The aim of this study is twofold: first, to offer a specific intervention proposal for pedophilia in the prison setting based on the available scientific evidence, and complementary to the Sexual Assault Control Program, and second, to justify this proposal through the case of an inmate with pedophilia. Specifically, we propose an individual intervention focused on people with pedophilia; working on low self-esteem, assessing suicidal ideation and restructuring cognitive distortions with minors, facilitating the creation of functional personal relationships with adults, assessing past polyvictimization and its possible influence on subsequent sexual behavior, providing inmates with psychoeducation on pedophilia; specifically addressing sexual fantasies with minors, avoiding dyssexual behavior, and eliminating the consumption of abusive material.(AU)

Background: Childhood institutional maltreatment (IM) is associated with both complex posttraumatic stress disorder (CTPSD) and poverty in adulthood life, each of which may impact an individual's quality of life (QoL). To find implications for clinical practice and policy making for adult survivors with childhood IM experiences, it is necessary to conduct research examining their current QoL and identifying related factors.Objective: By applying the model of the conservation of resources theory, we focused on how adulthood QoL can be indicated by childhood IM as well as the life outcomes of IM such as additional lifetime trauma, CPTSD, and poverty.Methods: In a cross-sectional study, self-report data were collected from 127 adults who were survivors of the 'Hyeongje' childhood IM in South Korea. We conducted regression analyses of childhood IM experiences, trauma experiences after escape from the institution, current CPTSD symptoms, and current poverty experiences on current QoL.Results: The duration of placement at the 'Hyeongje' (ß = .24, p = .009) was associated with trauma experiences after escape from the institution. Trauma experiences after escape from the institution (ß = .25, p = .007) were associated with CPTSD symptoms. CPTSD symptoms (ß = .26, p = .005) were associated with poverty, and both CPTSD symptoms (ß = -.52, p < .001) and poverty (ß = -.26, p = .003) were negatively associated with current QoL.Conclusions: Prolonged childhood IM brings about loss spirals by increasing an individual's exposure to experiences of further cumulative trauma, CPTSD, and poverty. There is a need for due diligence-based policy making and public support from the government to help create upward spirals for QoL. This may include the imminent detection and rescue of children as well as providing a safe environment, offering multidisciplinary interventions including evidence-based treatment for CPTSD, and considering economic support including collective reparations.


Duration of placement at the 'Hyeongje' institution was associated with additional trauma experience after escaping the institution.Cumulative trauma after escape was associated with CPTSD symptoms; CPTSD symptoms were associated with poverty; and both CPTSD symptoms and poverty were associated with poor current QoL.Multidisciplinary interventions including evidence-based treatment for CPTSD and considering economic resources for childhood IM survivors would be crucial in increasing QoL.

Introducción: El abuso sexual infantil es un problema social global y multidimensional que provoca resultados devastadores y permanentes en las relaciones psicológicas, emocionales, cognitivas, conductuales, físicas, sexuales e interpersonales. Este estudio examina la relación entre la capacidad de decir «no» y la conciencia de madres y padres sobre el abuso sexual en estudiantes de 4.° de primaria. Métodos: El estudio se realizó entre abril de 2022 y junio de 2022 en escuelas primarias de un distrito central provincial en el noreste de Turquía. La muestra del estudio estuvo formada por 310 alumnos de 4.° de primaria y sus madres y padres. Compilamos los datos del estudio con el Formulario de información personal, la Escala de capacidad para decir «no» para niños y la Escala de conciencia de abuso sexual para padres. Resultados: Hubo una correlación positiva débil (p˂0,05) entre las puntuaciones medias de conciencia de las madres sobre el abuso sexual y las puntuaciones medias de rechazo y resistencia de los niños, y una correlación positiva débil (p˂0,05) entre las puntuaciones medias de la conciencia de los padres sobre el abuso sexual y las puntuaciones medias de rechazo de los niños. Conclusión: A medida que aumentó la conciencia de las madres y los padres sobre los mitos y las enseñanzas sobre el abuso sexual y las acciones para combatir el abuso sexual, también aumentó la negativa de los niños. Se encontró que a medida que aumentaba la conciencia de los padres sobre las señales de abuso sexual, aumentaba la negativa de los niños.(AU)


Introduction: Child sexual abuse is a global and multidimensional social problem and causes devastating and permanent psychological, emotional, cognitive, behavioural, physical, sexual and interpersonal sequelae. This study examines the relationship between the ability to say «no» and parental awareness of sexual abuse in 4th grade primary school students. Methods: The study was conducted between April 2022 and June 2022 in primary schools in the central district of a province in north-eastern Turkey. The sample consisted of 310 students enrolled in 4th grade and their parents. We collected the data through a personal information form, the ability to say «no» scale for children and the sexual abuse awareness scale for parents. Results: There was a weak positive correlation between the mean maternal scores of sexual abuse awareness and the mean scores of refusal and resistance in children (P˂.05), as well as a weak positive correlation between the mean paternal scores of sexual abuse awareness and the mean scores of refusal and resistance in children (P˂.05). Conclusion: As mothers’ and fathers’ awareness of sexual abuse myths and of teachings and actions to combat sexual abuse increased, the refusal of children also increased. Also, as fathers’ awareness of the signs of sexual abuse increased, children's refusal increased.(AU)

OBJECTIVE: Elder abuse, an important human rights issue and public health problem, contributes to increased disability and mortality. In the last decades, several reviews have synthesized primary studies to determine its prevalence. This umbrella review aimed to estimate the worldwide overall prevalence rate of elder abuse in the community and care setting. METHODS: Following prospective registration at PROSPERO (CRD42021281866) we conducted a search of eight electronic databases to identify systematic reviews from inception until 17 January 2023. The corrected covered area was calculated to estimate the potential overlap of primary studies between reviews. The quality of the selected reviews was assessed using a modified AMSTAR-2 instrument. We extracted data on the prevalence of any type of elder (people aged 60 years old or older) abuse in the community and care setting. RESULTS: There were 16 systematic reviews retrieved between 2007 and 2022, out of which ten captured prevalence globally, three in Iran, one in Turkey, one in China and one in Brazil. The 16 reviews included 136 primary studies in total between 1988 and 2020. The overlapping of studies between reviews was found to be moderate (5.5%). The quality of reviews was low (2, 12.5%) or critically low (14, 87.5%). The estimated range of global prevalence of overall elder abuse was wide (1.1-78%), while the estimations of specific abuse prevalence ranged from 0-81.8% for neglect, 1.1-78.9% for psychological abuse, 0.7-78.3% for financial abuse, 0.1-67.7% for physical abuse, and 0-59.2% for sexual abuse. CONCLUSIONS: Although the low quality of the evidence and the heterogeneity of the phenomenon makes it hard to give precise prevalence data, it is without a question that elder abuse is a prevalent problem with a wide dispersion. The focus of attention should shift towards interventions and policymaking to prevent this form of abuse.

INTRODUCTION: Child sexual abuse is a global and multidimensional social problem and causes devastating and permanent psychological, emotional, cognitive, behavioural, physical, sexual and interpersonal sequelae. This study examines the relationship between the ability to say "no" and parental awareness of sexual abuse in 4th grade primary school students. METHODS: The study was conducted between April 2022 and June 2022 in primary schools in the central district of a province in north-eastern Turkey. The sample consisted of 310 students enrolled in 4th grade and their parents. We collected the data through a personal information form, the Ability to Say "No" Scale for Children and the Sexual Abuse Awareness Scale for Parents. Background: Childhood maltreatment profoundly influences social and emotional development, increasing psychiatric risk. Alterations in the implicit processing of threat-related cues following early abuse and neglect represent a marker of mental health vulnerability. Less is known about how early adversity influences the perception of positive social cues, despite their central role in establishing and maintaining social interactions and their association with better mental health outcomes.Methods: The sample consisted of 42 children and adolescents with substantiated childhood maltreatment experiences and 32 peers (mean age 13.3), matched on age, pubertal status, gender, socioeconomic status, ethnicity, and cognitive ability. A computerised experimental task assessed the perceived emotional intensity of positive (happy) and negative (fearful) facial expressions. Mental health symptoms were measured via self- and parental reports, and perceived social support was self-reported.Results: The experience of abuse and neglect was associated with heightened perceived intensity of positive facial cues. Cross-sectional post-hoc moderation and mediation analyses, employing a model-building approach, revealed that in maltreatment-exposed participants: (i) their increased response to positive facial cues was associated with lower symptoms; (ii) the presence of social support accounted for their heightened perceived intensity of positive facial cues; (iii) the presence of social support putatively contributed to lower symptoms by increasing the perceived intensity of positive facial cues. No group differences in perceived intensity of negative expressions were observed.Conclusions: These findings provide fresh insight into how positive faces are processed following maltreatment experience in childhood. Maltreatment experience was found to be associated with heightened perceived intensity of happy faces, which in turn was associated with better mental health and greater levels of social support. This suggests that heightened saliency of positive emotions acts protectively in children with maltreatment experience.


The impact of childhood maltreatment on perceiving positive social cues, especially explicit emotional intensity appraisal, is less understood compared to the extensive focus on its role in processing threat-related cues and psychiatric risk.In this study, we found that maltreatment experience was associated with heightened perceived intensity of happy faces, which in turn was associated with better mental health and greater levels of social support.These findings suggest that heightened saliency of positive emotions may act protectively in children with maltreatment experience.

Treatment Rationale: Exposure to repeated sexual trauma, particularly during childhood, often leads to protracted mental health problems. Childhood adversity is specifically associated with complex posttraumatic stress disorder (PTSD) presentation, which is particularly tenacious and treatment refractory, and features severe emotion dysregulation. Augmentation approaches have been suggested to enhance treatment efficacy in PTSD thus integrating first-line psychotherapy with mechanistically informed self-neuromodulation procedures (i.e. neurofeedback) may pave the way to enhanced clinical outcomes. A central neural mechanism of PTSD and emotion dysregulation involves amygdala hyperactivity that can be volitionally regulated by neurofeedback. We outline a treatment rationale that includes a detailed justification for the potential of combining psychotherapy and NF and delineate mechanisms of change. We illustrate key processes of reciprocal interactions between neurofeedback engagement and therapeutic goals.Case Study: We describe a clinical case of a woman with complex PTSD due to early and repetitive childhood sexual abuse using adjunctive neurofeedback as an augmentation to an ongoing, stable, traditional treatment plan. The woman participated in (a) ten sessions of neurofeedback by the use of an fMRI-inspired EEG model of limbic related activity (Amygdala Electrical-Finger-Print; AmygEFP-NF), (b) traditional weekly individual psychotherapy, (c) skills group. Before and after NF training period patient was blindly assessed for PTSD symptoms, followed by a 1, 3- and 6-months self-report follow-up. We demonstrate mechanisms of change as well as the clinical effectiveness of adjunctive treatment as indicated by reduced PTSD symptoms and improved daily functioning within this single case.Conclusions: We outline an integrative neuropsychological framework for understanding the unique mechanisms of change conferring value to conjoining NF applications with trauma-focused psychotherapy in complex PTSD.


Self-neuromodulation procedures that regulates limbic-related activity in adjunction to therapy show clinical effectivity in complex PTSD.We present an integrative perspective of neurofeedback embedded in psychotherapy, illustrated by a single case report.A single case provides an illustration of the potential utility of multifaced treatment including psychotherapy with adjunctive neurofeedback.

OBJETIVO: Realizar a análise do conceito de abuso sexual em crianças, identificando os possíveis atributos, antecedentes e consequências. MÉTODO: Foi conduzida uma análise de conceito, estruturada por meio de uma scoping review. A busca foi realizada em 6 bases de dados e, após análise, um total de 17 artigos foram incluídos. RESULTADOS: Foram identificados, como antecedentes, crianças com idade acima de 8 anos, do sexo feminino, sendo o agressor frequentemente um parente da vítima ou até mesmo o próprio pai. Quanto aos atributos, foram identificadas alterações psicológicas, doenças sexualmente transmissíveis e gravidez indesejada. As consequências observadas foram predominantemente de natureza psicológica, incluindo depressão e comportamento suicida. CONCLUSÃO: No total, foram agrupados nove antecedentes que estão relacionados aos fatores que levam à vulnerabilidade do abuso. Além disso, foram identificados 11 atributos correspondentes aos sinais característicos que indicam que uma criança é vítima de abuso. Por fim, foram observadas 12 consequências decorrentes da violência sexual infantil.


OBJECTIVE: To conduct a conceptual analysis of child sexual abuse to identify possible attributes, antecedents, and consequences. METHOD: A concept analysis was conducted using a scoping review methodology. Searches were conducted in 6 databases, resulting in 17 articles after analysis. RESULTS: Antecedents included children older than 8 years, female gender, the perpetrator often being a relative of the victim or even the biological father. Psychological changes, sexually transmitted diseases, and unwanted pregnancies were identified. The consequences observed were mainly psychological, including depression and suicidal behavior. CONCLUSIONS: Nine antecedents were grouped as factors contributing to vulnerability to abuse. In addition, 11 attributes correspond to characteristic signs indicating that a child is a victim of maltreatment. Finally, 12 consequences of child sexual violence were observed.

La pandemia por covid19 ha generado muchas publicaciones acerca de su influencia en el tratamiento y evolución de personas con un trastorno por uso de sustancias, con resultados contradictorios, a veces basadas en datos y otras en inferencias indirectas de otros datos. En este trabajo se estudia la adherencia y tasas de abstinencia de pacientes que inician tratamiento en una Unidad de Conductas Adictivas durante la pandemia, respecto a otros que acudieron el año previo y comparando los que hicieron visita presencial o telefónica. Los resultados indicaron mejor adherencia al mes de seguimiento en los pacientes del periodo covid19 y en los que fueron atendidos telefónicamente. A los 3 y 12 meses se mantuvo una mejor adherencia, aunque no significativa estadísticamente. Respecto a la abstinencia, el pequeño tamaño de la muestra dificultó obtener diferencias significativas. Se concluye que la pandemia, aunque ha significado una disminución del número de inicios de tratamientos por consumo de sustancias, también ha repercutido en una mayor adherencia a corto y medio plazo. La atención telefónica puede jugar un papel importante y positivo en este aspecto, complementario a otros recursos e intervenciones. (AU)


The Covid19 pandemic has led to many publications about its influence on the treatment and evolution of individuals with a substance use disorder, leading to contradictory results. In this study, the adherence and abstinence rates of patients who started treatment in an Addictive Behavior Unit during the pandemic are analyzed, compared with others who attended the previous year and comparing those who were attended in person or by phone. The results indicate that during the Covid19 period, patients had greater adherence to treatment after one month of follow up and when attended to by phone. At 3 and 12 months, greater adherence was maintained, although it was not statistically significant. Regarding abstinence, the small sample size made it difficult to obtain significant differences. The conclusion is that, despite a quantitative decrease in the number of patients beginning drug treatment, in qualitative terms the pandemic led to greater adherence in the short and medium term. Telephone attention can play an important and positive role at this point, complementary to other resources and interventions. (AU)

La adherencia terapéutica, es un hecho variante y complejo ya que son diversos los factores que influyen en él. En los países desarrollados la adherencia terapéutica de los pacientes que padecen enfermedades crónicas es del 50%, requiriendo acciones de prevención y promoción de la salud, que se ven intensificadas cuando la falta de adherencia terapéutica está unida al abuso de sustancias y la dependencia de alcohol. El señor D.R. es un paciente varón de 69 años con paresia crural izquierda, trastorno de la marcha, dependencia para las ABVD y AIVD, HTA, ateromatosis carotidea, mala adherencia terapéutica, abuso de alcohol, sobrepeso e hipercolesterolemia, que ingresa en el hospital en el servicio de Neurología tras sufrir un ictus isquémico. Durante este ingreso, se distingue el progreso en el nivel de implicación y compromiso del paciente con su propia salud. A través de la taxonomía NANDA-I, NOC y NIC, se llevó a cabo un plan de cuidados, aplicando un lenguaje estandarizado y con intervenciones basadas en la evidencia científica. (AU)

Purpose: A substantial increase in the prescription of immediate release fentanyl (IRF) outside hospitals was observed in previous studies between 2012 and 2017, however it remains unknown the extent of immediate release fentanyl use disorders (IRFUD). This study aimed to estimate the incidence and risk factors of IRFUD, such us abuse and dependence, in Spain during this period. Methods: Retrospective cohort study performed in a Spanish electronic primary care healthcare records database (BIFAP). The incidence rate of IRFUD was calculated by dividing the number of incident cases by the total patient-years (p-y) of exposure. Demographic data, lifestyle, cancer diagnosis, comorbidities and concomitant medication were described and analyzed overall and in patients developing IRFUD using Cox regression models. Effect of the type of treatment (continuous/discontinuous) and duration were also evaluated. Results: The incidence of IRFUD in the 12,267 patients analyzed was 1.8 cases per 100 p-y of exposure. Baseline analysis showed higher frequencies of IRFUD for smokers, patients with a history of substance abuse, non-oncology indication and diagnosis of depression and anxiety, respect to non-IRFUD patients. Patients aged ≥ 80 were less likely to develop IRFUD abuse/dependence. Significant differences were for concomitant use of other treatments with potential for dependence and abuse, such as benzodiazepines. The risk of IRFUD increased with treatment duration, being the highest for treatments lasting 180 days and longer. Conclusion: Incidence of IRFUD is difficult to contrast due to the lack of similar studies. It could be considered not too higher outside hospitals but possible in cancer and non-cancer patients... En estudios previos se ha observado un aumento considerable en la prescripción de fentanilo de liberación inmediata en el ámbito extrahospitalario entre 2012 y 2017. Sin embargo, aún se desconoce la magnitud de dependencia y abuso derivada de su uso. Este estudio tiene como objetivo estimar la incidencia y los factores de riesgo de los trastornos derivados del uso de fentanilo de liberación inmediata en España, tales como el abuso y la dependencia durante este período. Métodos: Estudio de cohortes retrospectivo realizado en España en la base de datos de historias clínicas electrónicas de atención primaria (BIFAP). La tasa incidencia de trastornos por el uso de fentanilo, se calculó dividiendo el número de casos incidentes entre el total de personas-año de exposición. Se analizaron datos demográficos, estilo de vida, diagnóstico de cáncer, comorbilidades y medicación concomitante. Para el análisis se utilizaron modelos de regresión de Cox. También se evaluó el efecto del tipo de tratamiento (continuo/discontinuo) y la duración de tratamiento. Resultados:La incidencia de trastornos por el uso de fentanilo en los 12,267 pacientes analizados fue de 1.8 casos por 100 personas-año de exposición. Se observó una frecuencia más elevada en aquellos pacientes que al inicio del tratamiento eran fumadores, pacientes con antecedentes de abuso de sustancias, con indicaciones no oncológicas y en pacientes con diagnóstico de depresión y ansiedad en comparación con los pacientes que no desarrollaron el evento. Los pacientes mayores de 80 años presentaron menos probabilidades de desarrollar estos trastornos. Se observaron diferencias significativas en el uso concomitante con otros tratamientos con potencial de desarrollar dependencia y abuso, como las benzodiacepinas. Background: Adverse childhood experiences (ACEs) have been shown to negatively affect health in adulthood. Estimates of associations between ACEs and chronic painful conditions are lacking.Objectives: This systematic review and meta-analysis aimed to evaluate associations between exposure to ACEs and chronic pain and pain-related disability in adults.Methods: We searched 10 electronic databases from inception to February 2023. We included observational studies assessing associations between direct ACEs (childhood sexual, physical, emotional abuse, or neglect) alone or in combination with indirect ACEs (witnessing domestic violence, household mental illness), and adult chronic pain (≥3 months duration) and pain-related disability (daily activities limited by chronic pain). Pairs of reviewers independently extracted data and assessed study risks of bias. Random-effect models were used to calculate pooled adjusted odds ratios [aOR]. Tau square [T2], 95% prediction intervals [95%PI] and I2 expressed the amount of heterogeneity, and meta-regressions and subgroup meta-analyses investigated sources of heterogeneity (PROSPERO: CRD42020150230).Results: We identified 85 studies including 826,452 adults of which 57 studies were included in meta-analyses. Study quality was generally good or fair (n = 70). The odds of reporting chronic pain in adulthood were significantly higher among individuals exposed to a direct ACE (aOR, 1.45, 95%CI, 1.38-1.53). Individuals reporting childhood physical abuse were significantly more likely to report both chronic pain (aOR, 1.50, 95CI, 1.39-1.64) and pain-related disability (1.46, 95CI, 1.03-2.08) during adulthood. Exposure to any ACEs alone or combined with indirect ACEs significantly increase the odds of adult chronic painful conditions (aOR, 1.53, 95%CI, 1.42-1.65) and pain-related disability (aOR, 1.29; 95%CI, 1.01-1.66). The risk of chronic pain in adulthood significantly increased from one ACE (aOR, 1.29, 95%CI, 1.22-1.37) to four or more ACEs (1.95, 95%CI, 1.73-2.19).Conclusions: Single and cumulative ACEs are significantly associated with reporting of chronic pain and pain-related disability as an adult.


Previous meta-analyses highlighted the negative impact of adverse childhood experiences on physical, psychological, and behavioural health across the lifespan.We found exposure to any direct adverse childhood experience, i.e. childhood sexual, physical, emotional abuse, or neglect alone or combined, increased the risk of reporting chronic pain and pain-related disability in adulthood.The risk of reporting chronic painful disorders increased with increasing numbers of adverse childhood experiences.

Objetivo: evaluar el uso de sustancias y la salud mental de los camioneros brasileños, y cómo estos impactan en sus comportamientos re-lacionados con la conducción. Métodos: estudiotransversal realizado con 235 camioneros en Brasil. Resultados: Los conductores que condujeron des-pués de beber eran fumadores, sufrieron agresiones y experimentaron an-siedad. Las fallas en la conducción se asociaron con la conducción después de beber y han sufrido agresiones. Los errores se asociaron con la altera-ción del sueño y la participación en accidentes que tuvieron víctimas. Las infracciones fueron mayores entre quienes condujeron después de beber, consumieron drogas, tenían ansiedad y problemas de sueño. Conclusión: El uso de sustancias, la salud mental, la somnolencia y la violencia están inter-relacionados para los camioneros brasileños.(AU)


Objective: to evaluate the substance use and mental health of Bra-zilian truck drivers, how these impact on their driving-related behaviors.Methods: cross-sectional study conducted with 235 truck drivers in Brazil. Results: The drivers who drove after drinking were smokers, suffered as-saults and experienced anxiety. Lapses in driving were associated with driv-ing after drinking and have suffered assaults. Errors were associated with impaired sleep and being involved in accidents that had victims. Violations were greater amongst those who drove after drinking, used drugs, had anx-iety and impaired sleep. Conclusion: Substance use, mental health, drowsi-ness and violence are interrelated for Brazilian truck drivers.(AU)

Background/Aim: Young people in care may show significant mental health difficulties because of previous adverse experiences, such as maltreatment or violence. However, no meta-analyses have been conducted on this topic. We aim to identify the role of previous maltreatment in current symptoms of children and adolescents in out-of-home care. Method: A search in Academic Search Complete, PsycArticles, PsycInfo, Psychology and Behavioral Sciences Collection, ERIC, Medline, Web of Science and Scopus databases was conducted. Studies were included if they were empirical and quantitative, involved school-aged children and adolescents, assessed maltreatment before placement in care and current internalizing and externalizing symptoms. Multiple effect sizes were extracted from each primary study, and a three-level meta-analysis for each type of maltreatment associated with internalizing and externalizing symptoms was conducted. Results: The search yielded 122 effects sizes from 28 studies. Results indicated significant overall effects of general maltreatment (r = .260), abuse (emotional/physical) (r = .135) and sexual abuse (r = .247) on internalizing symptoms. In relation to externalizing symptoms, overall effects of abuse (emotional/physical) (r = .097) and sexual abuse (r = .187) were identified. The overall effect sizes of neglect were neither significant for internalizing or externalizing symptoms. A set of moderators was tested, and significant effects were found for the type of maltreatment measure on internalizing symptoms and for gender on externalizing symptoms. Conclusions: This study provides new insights beyond previous systematic reviews, as we were able to disentangle the associations between maltreatment and internalizing and externalizing symptoms in out-of-home care. Antecedentes/Objetivos: Los niños y adolescentes de protección presentan problemas significativos en la salud mental debidos a experiencias previas adversas como el maltrato o la violencia. A pesar de ello no se han realizado meta-análisis sobre esto. Se diseñó una revisión meta-analítica con el objetivo de conocer el papel que desempeña el maltrato sufrido en los síntomas que experimentan los niños y adolescentes en acogida fuera del hogar. Método: Se llevó a cabo una búsqueda bibliográfica en las bases de datos Academic Search Complete, PsycArticles, PsycInfo, Psychology y Behavioral Sciences Collection, ERIC, Medline, Web of Science y Scopus. Se incluyeron los estudios que eran empíricos y cuantitativos, concernían a niños y adolescentes en edad escolar, evaluaban el maltrato antes de que fuesen dados en acogida y también los síntomas actuales internalizantes y externalizantes. De cada estudio primario se extrajeron varios tamaños del efecto y se ejecutaron meta-análisis de tres niveles para cada tipo de maltrato asociado con síntomas internalizantes y externalizantes. Resultados: La búsqueda arrojó 122 tamaños de efecto provenientes de 28 estudios. Los resultados mostraron efectos generales significativos del maltrato general (r = .260), el abuso (físico/emocional) (r = .135) y el abuso sexual (r = .247) en los síntomas internalizantes. En cuanto a los síntomas externalizantes se encontraron efectos globales del abuso (emocional/físico) (r = .097) y abuso sexual (r = .187). Los tamaños del efecto globales de la negligencia fueron no significativos tanto ni para los síntomas internalizantes como para los externalizantes. También se estudiaron los efectos de moderadores, hallándose efectos significativos del tipo de medida del maltrato en los síntomas internalizantes y del género en los síntomas externalizantes. Objetivo: Analizar avances en la literatura científica sobre factores de riesgo y sintomatología a corto y largo plazo del abuso sexual en niños, niñas y adolescentes. Método: Mediante una revisión de artículos científicos en inglés y en español, se seleccionaron 27 investigaciones disponibles en APA PsycArticles, Dialnet, Springer y ScienceDirect, entre 2011 y 2020 con información cuantitativa o cualitativa sobre factores de riesgo y sintomatología del abuso sexual en niños, niñas y adolescentes. Resultados: Se identifican factores de riesgo asociados a grupos etarios, principalmente entre 6-7 y 12-13 años, tipos de familias reconstituidas, así como a la presencia de cuidadores distintos a los padres. Entre los síntomas a corto plazo, se identifican cambios bruscos de comportamiento, dificultades en las relaciones interpersonales, angustia, miedo y culpa. Además, se registran lesiones físicas y alteraciones somáticas. A largo plazo se identifican síntomas asociados a estrés postraumático, conductas suicidas, trastornos alimenticios, dificultades de interacción social y en menor medida drogodependencia. Conclusiones: Respecto a los factores de riesgo se visibiliza el uso de redes sociales como un referente a considerar en las dinámicas actuales. Sin embargo, la literatura enfatiza en seguir consolidando el análisis de estos factores y su articulación con acciones pertinentes para la prevención y atención oportuna del abuso. En cuanto a la sintomatología se evidencian avances en el análisis a corto plazo, pues no sólo se aborda lo físico y fisiológico sino también desde lo psicosocial. Similarmente, el análisis de la sintomatología a largo plazo se orienta principalmente a procesos psicosociales.


Objective: To analyze current scientist literature advances about risk factors and symptomatology of sexual abuse in children and adolescents. Method: A literature review was conducted by consulting english and spanish documents available in APA PsycArticles, Dialnet, Springer and Science Direct databases. 27 research papers published between 2011 and 2020 was selected. They provide quantitative or qualitative information about risk factors and symptomatology of sexual abuse in children and adolescents. Results: Main findings describe risk factors associated with age groups, mainly between 6-7 and 12-13 years, types of reconstituted families, as well as the presence of caregivers different to parents. Short-term symptoms include abrupt changes in behavior, difficulties in interpersonal relationships, anguish, fear and guilt. In addition, physical injuries and somatic alterations are identified. In the long term, symptoms are associated with post-traumatic stress, suicidal behaviors, eating disorders, difficulties in social interaction and sometimes with drugs abuse. Discussions: Regarding risk factors, the use of social media is visible as a reference to be considered in current dynamics. However, the literature emphasizes the need to continue consolidating the analysis of these factors and their articulation with pertinent actions for the prevention and timely attention of abuse. Regarding symptomatology, there is evidence of progress in the short-term analysis, since not only the physical and physiological aspects are addressed, but also the psychosocial aspects. Similarly, the analysis of long-term symptomatology is mainly oriented to psychosocial processes.
